• Any major violation within a 36-month period.

• Three or more accidents (regardless of fault) in the last 36 month period.

• Any combination of accidents and moving violations which is equal four or more in the last 36 months period.

• Any drug or alcohol related violation in the past 5 years.

• Less than two years of consistent commercial driving experience.

• Being under 23 years of age.

• Being over 75 years of age.

• If the CDL is issued less than 2 years ago copy of the previous CDL will be required.

Independent contractor’s must be a minimum age of 23 years old

• Independent contractors cannot be 75 years old or older

• Independent contractor s 65 years of age of older are required to submit a long medical form with a passing grade of good health

• Independent contractors must have proof of a minimum of 2 years consistent CDL experience. Acceptable documentation to confirm this are as follows…

o PSP report that can verify a minimum of 2 years’ experience

o Work History – DEH

o Copy of previous license showing issue date more than 24 months ago

o Copy of previous MVR that shows the prior issue date of the CDL

• CDL must be valid

• Driving while intoxicated

• Driving under the influence of drugs

• Negligent homicide arising out of the use of a motor vehicle

• Using a motor vehicle for the commission of a felony

• Aggravated assault with a motor vehicle

• Permitting an unlicensed person to drive

• Reckless Driving

• Speed contest

• Hit and Run (BI and PD) driving
